Patient consultation: please allow at least one hour for your initial appointment.
While our goal is to see patients at their scheduled appointment time, our providers can occasionally be called into emergency surgery or to see a patient with a medical emergency. If an emergency arises, we will do our best to notify you prior to your appointment, however that is not always possible. Your patience and understanding are greatly appreciated.
- Please bring with you a photo ID and insurance card to every appointment
- Certain patients will also need to provide a copy of their X-rays (CD-ROM Format)
- Bring a list of ALL your medications
If you have a more complicated or involved issue that has already been evaluated by any other physician, it is best that you have ALL your pertinent records sent to our office at least 2 weeks in advance of your appointment for review. This information would pertain to Laboratory, Radiology, Pathology, Surgical Treatment, and medications used in the treatment process.
Financial Policy and Procedures
For your convenience, we accept Visa and MasterCard, cash or checks. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at 812-669-1454. Many times, a simple telephone call will clear any misunderstandings.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan, but that must be implemented prior to the actual procedure.

Insurance Information
As a patient of this practice, you must understand your rights and obligations. Professional services are only billed to third party payors in accordance with standard office practices and third party policies. This holds true to medical and surgical services provided here and outside this office including outpatient and inpatient surgery.
The insurance company will only pay for treatment AFTER services had been provided, including surgery. It is very important that you understand that when the insurance pre-certifies treatment, it does not necessarily guarantee that the insurance company will pay for all of your bills. Your insurance company may deny payment based upon non-covered benefits, pre-existing conditions, lack of benefits or other reasons that you should discuss with your insurance company directly for full disclosure. In addition part of your charges may be applied to your deductible, and you are liable for these charges according to your agreement with your insurance company.
Our staff will obtain the pre-certification when necessary for any tests and surgery that are scheduled for you, but we may need additional information from you or other physicians to document any previous treatment. Please sign a separate release for those records if asked to do so.
BE AWARE: We can in no way guarantee to you that your insurance will cover the cost of your treatment. It is always your responsibility to understand your current insurance benefits and to discuss this with your insurer prior to receiving treatment. You will ultimately be responsible for the balance of any unpaid charges.
If you have a co-pay and/or have not met your deductible, the co-pays or the amount of money to meet such deductible is due at time of service and prior to surgery. Any and all over payments will be returned to you after claims are processed without accruing interest.
